• Overall Dive & Trip Insurance information - Explorer Ventures strongly encourage all guests to secure insurance prior to diving and traveling. Dive accident insurance is required for the Galapagos & Maldives.
• Medical Form (PDF) - Recreational Scuba Training Council
• Nitrox Waiver (PDF) - required to be signed on board prior to diving with nitrox. Copies are available on board as well; we do not need to receive this form in the booking office prior to your trip
